ICN Drop/Withdrawal Form

The ICN Drop/Withdrawal Form may be used to notify the campus coordinator at your Home Institution that you are dropping or withdrawing from a course. To print, click on the printer icon located in the upper right corner of the form. Be sure to keep a copy of the completed form for your records.
